Stay on top of your health with My Care – Jordan’s Story

Jordan is a Principle Trainer for the social care app within the encompass project who has found great use for the My Care patient portal as a person who is also living with type 1 diabetes.

Since My Care was introduced in the Northern Trust, Jordan says his experience of managing appointments and clinics has been transformed. He can now:

  • Keep track of appointments – No more worries about missing important dates.
  • Access after-visit summaries – Making it easy to review key information once he gets home which he may have missed otherwise.
  • Contact his diabetes specialist nurse – Jordan has found it quick and easy contact his care team if he has any questions or concerns.
  • View medication details – This help Jordan keep track of any important changes.

For Jordan, who attends regular and sometimes lengthy diabetes clinics, My Care has made a real difference. Instead of worrying about forgetting information or keeping track of notes, everything he needs is in one secure and accessible place.

Jordan said: “For me, My Care has changed my life dramatically from an appointments perspective. I’m always confident that I have the right information, and I know I can reach out to my care team when I need to.”

Jordan’s story is just one example of how the My Care patient portal is helping people take more control of their health and stay connected with their care teams.

My Care is only available within secondary care (your specialist in the hospital or in the community setting). This means primary care (GP records) are not available.
Social Care records are currently not available but will be added in the future.
